Where: step 1 of setup (The school), when tapping Continue.
Why: the account already has a school. In V1, a person belongs to one school only. This mostly happens when setup was open in two tabs, or after a double click: the school was created once.
What to do: reload the page: setup resumes with the existing school, at the step reached. Nothing was created twice.
